With respect to health consults with our licensed health care providers and therapists, you will be asked to fill out a detailed case history form prior to your first visit. This medical information is vital to assess the risks and to determine the appropriate treatment precautions.
All case history files are kept in a secure area and viewed only by your healthcare provider and/or therapist. You are free to access information in your files; copies are also available at your request for a nominal fee.
Files are not shared with any other promotional agencies or other institutions without your direct permission. This is in adherence to the privacy policy mandate for client information confidentiality.
If you have any questions or concerns, please discuss them with your healthcare provider and/or therapist prior to your treatment.
Natural Health Products
Because the function of the human body is complex, natural health products (nutrients, herbal extracts, vitamins/minerals) should not be assumed to be safe. They can lead to adverse effects due to interactions with certain medical conditions or prescription medication.
